Insurance

IF YOU DO NOT HAVE INSURANCE

- The fee is to be paid in full on the day of your appointment unless other arrangements are made with us. If you cannot pay the fee in full, we ask that a minimum payment of one-forth (1/4) of the fee be made before you leave our office. If you cannot meet this payment schedule, please visit with the front desk personnel to make payment arrangements that will fit your financial situation.

IF YOU HAVE INSURANCE

- A payment of twenty percent (20%) of the total fee is required at the first visit. After completion of the root canal therapy, as a courtesy, we will file your insurance claim for you. If there is a remaining balance due after receipt of the insurance payment, we will send you a statement. That stament is expected to be paid in full unless arrangements are made with us for continuing monthly payments. Knowing your yearly insurance limit is your responsibility. Please advise us of any insurance changes.
