For Root Canal Therapy Our usual fee is established
according to the location and complexity of the evaluation and treatment
required: Anteriors (Front Teeth), Bicuspids (Middle Teeth), Molars (Back
Teeth), and the Permanent Filling in the Access opening.
Retreatment of
previous root canal therapy and surgery treatment fees are separate.
For an Evaluation The fee
has been established according to the location and number of teeth examined and
the nature of the diagnostic procedures.
Your Appointment We ask that you arrive 15 minutes before
your appointment time to complete a short health history. If one year has
passed since your last visit in the office, you will need to complete another
health history form. Click here for our
Health History PDF Form that you can print out and complete before you
arrive. If you are taking several prescriptions, please bring a list for us to
incorporate into your record for this visit. If a patient is under the age of
18 years of age, a parent or legal guardian will need to be present with the
patient to approve the work to be done.
Insurance Please bring all insurance information so that
we may file your insurance as a courtesy. It is helpful if you bring an
insurance card at the time of your appointment. Endodontics, P.C. participates
with Delta Dental Premier, Blue Dental, Principal, Aetna, Cigna, Ameritas, Hawk
I, and DeCare Dental Teamsters program. The fee paid by an insurance company is
the amount of your policy limits for root canal therapy. Customarily, the fee
paid is from 50% to 80% of our fees. You are responsible for the balance after
insurance. Please call your insurance company or your employer administrator if
you are unsure of your policy limits. |
